These questionnaires are designed to complement active therapy sessions and should only be utilized within the context of ongoing therapeutic treatment. All responses from these questionnaires are confidential and will be shared directly with the therapist overseeing your care. Please note that these assessments are intended for use as part of a comprehensive therapeutic approach and not for standalone use or self-diagnosis.
The following questionnaires are validated assessment tools widely used in clinical practice. They help evaluate different aspects of your mental health and well-being. Please complete relevant questionnaires before our session to help guide our discussion.
Pre-Discovery Questionnaire
Initial questionnaire to gather essential basic information to help guide our therapeutic journey together.
Beck Anxiety Inventory (BAI)
A self-report questionnaire designed to measure the severity of anxiety symptoms experienced by an individual. Developed by Aaron T. Beck to assess both physiological and cognitive symptoms of anxiety.
Beck Depression Inventory (BDI-II)
A 21-item self-report inventory measuring cognitive, affective, and somatic symptoms of depression. Rate symptoms experienced over the past two weeks on a scale from 0 to 3.
Maslach Burnout Inventory (MBI)
A comprehensive assessment of burnout symptoms in professional settings, measuring emotional exhaustion, depersonalisation, and reduced personal accomplishment.
Psychological Well-being Scale (PWS)
Evaluates various aspects of psychological well-being and functioning, including positive affect, life satisfaction, and personal growth across different contexts.
Perceived Stress Scale (PSS)
Assesses how individuals perceive situations in their life as stressful during the past month, evaluating stress levels across various life domains.
Secondary Trauma Stress Scale (STSS)
Measures the impact of exposure to traumatic experiences in caregivers and professionals, evaluating secondary trauma and compassion fatigue.
How to Use These Assessments
- Choose the questionnaires that are most relevant to your current concerns
- Complete the assessments honestly for the most accurate results
- Take note of any patterns or changes in your responses over time
- We will discuss your results during our sessions to inform our therapeutic work
